We are proud to be the leading provider of short-term skilled nursing and rehabilitation care and long-term living in Greene County. Our Homestead memory care neighborhood is a brand new, secure area specifically designed to improve the quality of life for individuals with Alzheimer’s, dementia, and other memory loss conditions. We understand how difficult it can be for families and aim to alleviate their stress and anxiety through our exceptional care.

Admission Information
We recognize that transitions can be a challenge for you and your family. The more information you have about the process, the better it is for all involved.
We encourage prospective residents and their families to schedule a tour and meet our knowledgeable staff. This will allow for a smooth transition from the hospital or home setting. Our Admissions Coordinator will diligently guide you through the admission and transfer process. Our Community Liaisons work closely with patients, families, hospital staff, and physicians to determine the most appropriate treatment and setting.
